Job summary

General Practice Nurse Post ( GPN foundation programme) BAND 5 eqiv - Minimum 30 hours a week Tues- friday( additional hours may be available on discussion). An exciting opportunity has arisen for a Nurse to join our friendly and dynamic clinical team here at Bron Derw Medical Centre. We are looking for a qualified nurse who is motivated and committed to becoming part of our team to pursue a career in General Practice Nursing. This post is ideally suited to newly or recently qualified nurses and no experience of primary care is required. We have a funded place allocated on the All Wales General Practice Nursing (GPN) Foundation Programme for the successful applicant. Together with the support of our experienced Nurse and ANP mentor, this programme will include the teaching and training required to develop into a General Practice Nurse with a wide skillset and able to effectively manage chronic diseases, including diabetes, COPD and Asthma. Main duties of the job

As part of the GPN programme, the trainee will develop the following core competencies to an independent level. Venepuncture, ear assessment, wound management, health promotion, health screening, cervical screening, childhood and routine immunisations, contraception, zoladex/ B12, denonsumab injections, travel health. With the exception of occasional autumn -winter saturday flu/ covid clinics this post does not include any work during out of hours or at weekends.

Canolfan Feddygol Bron Derw Medical Centre is a purpose built GP practice located in the historic town of Bangor North Wales. We have a population of 10,000 plus patients with a partnership of 3 GP partners supported by 2 salaried GP's, Nurse Practitioners, Allied health professionals, Nursing team, Practice Manager and a team of receptionists and administrators. We are a well established teaching practice supporting Nurse Practitioners trainees , Cardiff and Manchester medical students, foundation Year 2 doctors and GP trainees. We are a family friendly practice supporting the local community of Bangor and surrounding areas within our catchment area

Patient Care

Assisting in routine tasks related to patient care as directed by Doctors and Senior nurse e.g., family planning, cervical cytology, childhood and adult immunisations, wound care, treatment room etc.

Performing basic investigations e.g., ECGs, respiratory disease management, diabetes management, chronic disease management, urinalysis etc.

Dealing with minor injuries; referring on appropriately.

Health Promotion

To advise, support and where appropriate, administer vaccinations for patients travelling abroad.

Provide general and specific health screening (within agreed guidelines) and referral to doctor as appropriate. .

Offering advice regarding lifestyles and referring where appropriate to colleagues.

Implement and participate in vaccination and immunisation programmes in line with local and national campaigns.

Assisting with procedures (usually with Doctors)

Contraceptive implants, IUD inserts / removals and pessary changes.

Chaperoning colleagues as requested.

Minor surgical operations.

Assist with medical assessments when required.

Administrative

Ensure accurate, contemporaneous and complete computer records of all patient contacts either in consultation or with telephone advice. Ensure that these records are consistent with legislation, policies and procedures.

Ensure accurate completion of all documentation associated with patient health care and registration with the practice.

Recognise the pivotal place of good communication within the practice. Demonstrate sensitive communication styles to ensure that patients are fully informed and consent to treatment. Communicate effectively with patients and carers recognising that style may need to be modified and adapted to different levels of patient understanding. Anticipate barriers to communication and take action to improve communication. Demonstrate open and effective communication with individuals and groups within the practice environment and with external stakeholders.

Prioritise, organise and manage personal workload in a manner that maintains and promotes quality and effectiveness.

Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/ vulnerable adult health procedures and local guidance

Be aware of the issues for GMS contract as affects patient clinical care and support the work of the Practices Contract Administrator to maximise Practice income.

Mentoring and teaching

Bron Derw is a training Practice welcoming nursing and medical undergraduate and postgraduate students. Therefore, the post holder is expected to co-operate and participate in their education in relation to the activities of the nursing area.

Provide support and advice to the health care assistants who work alongside the qualified nursing staff.

Support and participate in shared learning across the practice and wider organisation.

Personal/Professional Development:

The post-holder is expected to maintain continued education by participating in any training in house or by attending external courses or study days deemed useful or necessary for their professional development and for the needs of the practice.

Attend and participate in relevant practice meetings; clinicians, audit/ clinical governance or nursing team administrative meetings.

To have the opportunity to participate in clinical supervision as required.

Participate in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development
