Job summary

We are looking for a Clinical Coder, Scanner and Medical Summariser to join our busy GP Practice working within our Administrative Team and taking responsibility for the coding and summarising of relevant information into our patients' healthcare records using the Vision clinical system.

We provide services to 10,000 patients in Aberystwyth and the surrounding areas and our Clerical Team is key to the smooth running of the practice. We can offer flexible working arrangements and a job share would be considered.

Main duties of the job

Key responsibilities include:

  • Accurately read-coding data onto patient records

  • Identifying clinical information, coding appropriately and completing detailed actions

  • Scanning of patient documentation and attaching scanned documents to patients' healthcare records

  • Amending and / or making additions to medical records to ensure that correct and complete information is each patients' electronic record

  • Opening and distributing post

  • Additional ad hoc tasks within the team as required

Previous experience in a medical setting is preferred but attitude is key in this role, we are looking for someone with these skills:

  • Organised and able to prioritise a busy and changing workload

  • Experience of administrative work

  • Flexible with a positive, can-do attitude

  • Discrete and able to maintain confidentiality

About Church Surgery

The Practice is conducted from a beautiful purpose built, converted church. We are fortunate to be situated on the Cambrian coast in the town of Aberystwyth which is surrounded by mountains. The Practice area is from approximately Furnace to the North, Llanon to the South and Cwmystwyth to the East. Our team includes six GPs, an ANP, three Practice Nurses, two Health Care Assistant, as well as our Practice Manager, reception, pharmacy technicians and administration staff. We are a training practice and support registrars, medical students, pharmacists and physicians associates.

Main purpose of post: The practice requires pertinent third party information to be read coded in to patients medical records to ensure the electronic record is up to date and accurately maintained

Hours of work: 37.5 hours per week

DUTIES & RESPONSIBILITIES:

The following list of duties should not be regarded as definitive as they may be reviewed from time to time to take account of changing circumstances. Also, tasks and duties may be redistributed amongst the practice team as necessary and cooperation should not be unreasonably withheld.

  • Review all incoming correspondence and enter, using read codes, pertinent information into the patients electronic medical record in accordance with the coding protocol. This will include data highlighted by clinicians as well as data required to meet contractual obligations.

  • Highlight clinician action and workflow to appropriate clinician or team as per Workflow Protocol.

  • Organise the medical record of patients whom you are coding, to harmonise problem titles, group symptom development and assign the appropriate status to medical information.

  • Recognise own knowledge limits and seek advice from clinical or administrative colleagues as appropriate.

  • You may be required to assist with note summarising in accordance with the summarising protocol.

  • To carry out administrative related tasks as delegated by Clinical Lead and Practice Manager.
