We are looking for a proactive and highly motivated person to become part of our friendly and excellent team. We are a training practice with registrars, medical and pharmacist students, having strong links to Swansea Medical School. The successful applicant will be encouraged to take an active role in development of the practice, becoming an active part of our MDT Care Home team, playing a key role in managing patient care within a portfolio of our local care homes. We are open to discussions about how individual roles might look to incorporate your specific interests and skills and to support your professional development.
Main duties of the job
Deliver of a high standard of primary care for the patients of the practice through assessment, diagnosis, treatment, and evaluation of care. Effectively manage patient's long-term conditions; they will be expected to process administration tasks, including referrals, prescription requests and other associated tasks; Work collaboratively, Contribute to the successful implementation of continuous improvements, development and quality initiatives within the practice; Attend and contribute to clinical and MDT meetings; Support the partners in achieving strategic aims, including enhancing income and reducing expenditure of the practice. We are a friendly 3 Partner, training practice working across 2 sites a mile apart at Cross Hands Health Centre and Tumble Surgery in the Amman Gwendraeth Valley. Currently we triage with F2F as and when it necessitates. We have a Clinical pharmacist, Advanced Nurse Practitioner and an experienced Nursing team on site working alongside an experienced and supportive admin team, managed by a Practice Manager, Assistant Practice Manager and a Reception Supervisor.
