Job summary
We are looking for a
motivated and compassionate Band 6 Community Care Coordinator to join the
Croydon Assessment and Liaison Team at South London and Maudsley NHS
Foundation Trust.
This is a varied and
rewarding role where youll work with individuals presenting with complex
bio-psychosocial needs, completing high-quality assessments, care planning, and
short-term interventions. Youll be part of a supportive multidisciplinary team,
working closely with partner agencies to ensure safe, effective, and
person-centred care. Youll also play a role in supporting and supervising
junior staff and students, and promoting service-user involvement across all
aspects of care delivery.
Main duties of the job
A&L is a
fast-paced but well-supported environment, offering a different experience from
a traditional CMHT. Caseloads are dynamic, with regular referrals and planned
discharges, and service users are typically supported for up to 12 weeks.
Clinicians hold a caseload alongside participating in a duty rota, which
includes screening referrals, managing the duty phone, and completing initial
screenings. The role offers excellent opportunities to develop strong
assessment, formulation, and decision-making skills within a collaborative
team.
