the Assertive Outreach and Rehabilitation Team. AORT is a dynamic
multi-disciplinary team dedicated to delivering high-quality assessment,
treatment, and rehabilitation for individuals living with complex mental health
needs, including co-occurring substance misuse, social exclusion, and high-risk
behaviours.
Main duties of the job
As part of this role, you'll coordinate and manage a caseload of clients using evidence-based, person-centred approaches. Youll work closely with a wide range of professionals across the Complex Care Pathway, participating in multidisciplinary activities and team quality improvement projects. This is a great chance to make a meaningful impact in a supportive and collaborative environment, where youll play a key role in empowering service users to achieve their recovery goals and live more independently.

Ward: Assertive Outreach Team Rehab 1 Branch
Pay Rates: Band 6, £21.96 per hour
Shift Patterns: Mon-Fri 9am-5pm
What you can add:
- Support service users recovery goals and independence, including those with physical health needs, by collaborating with local services and primary care teams.
- Provide clinical leadership and professional support to junior staff, acting as a role model and supporting their development through supervision.
- Build and maintain strong working relationships with community, inpatient, and external partner services to promote seamless care transitions.
- Understand and navigate placement processes and discharge pathways to reduce delays and improve patient outcomes.
- Maintain accurate, up-to-date clinical records, including risk assessments and contingency planning, in line with Trust policy.
In addition to experience, all Qualified roles will be required to provide evidence of:
- Relevant Qualifications
- Registration with the relevant Registration Body if applicable.
