Main duties of the job
Abstracts, analyses, translates and enters, codes based on medical information from patient records.
Deal with enquiries; liaise with clinicians regarding coding queries.
Locates and collates missing source material e.g. missing case notes or electronic documentation, histology or data quality records.
Achieves the performance requirements of an Accredited Clinical Coder.
Passes Clinical Coding Audit in all coding areas and works towards exceeding audit requirements.
Applies knowledge of complex national coding standards in order to assign codes using the ICD and OPCS classifications to record all diagnoses, procedures and interventions for in-patient, day case and regular day/night admissions, although this will be monitored (through audit), they will be expected to code areas they have previously been signed off in independently.
Works independently (without supervision) within all specialties.
Follows defined coding processes for resolving queries on their own.
Selects own workload to and knows when to use the internal query process to escalate more complex issues to senior members of staff or clinicians.

What youll be responsible for:
Working independently (without supervision) within all specialties.
Following defined coding processes for resolving queries on their own.
Selecting own workload to and knows when to use the internal query process to escalate more complex issues to senior members of staff or clinicians.
Is responsible for the provision of a high-quality clinical coding service. Abstracting and analysing documented medical terminology, then translating into clinical codes that can be used to give an accurate record of inpatient (Elective, Day Case and Emergency) hospital admissions.
Analysing medical terminology in terms of diagnostic and procedural information from patient documentation (The Trusts Scanning System, various clinical systems and paper case notes) in order to extract all relevant information to accurately reflect the patients stay in hospital.
Performing advanced Clinical Coding that requires the post holder to make analytical judgements regarding a wide range of complex patient documentation and utilising clinical coding conventions.
