Main duties of the job

Barnet South Core Team are a friendly and supportive team of
Community Therapists and Therapy assistants working with service users who have
complex and substantial needs in the Edgware, Middlesex area.

Responsibilities
Main Responsibilities:
- You will hold a case load across Barnet Community Core Hub to provide assessment, plan and implement interventions, evaluate treatment and ensure safe discharge.
- You will work closely with the Occupational Therapist and Therapy lead.
- You will work with service users who have complex and substantial needs, often working with people with dual diagnosis and co-existing conditions including physical health needs.
- You will work closely with multi-disciplinary teams and relevant outside agencies.
To be successful in applying for this role you will need:
- HCPC registration
- Degree in Occupational Therapy
- Understand MOHO ax tools
- Demonstratable experience in completing ADL reports
