Hertfordshire Community Trust (HCT) are seeking a highly motivated and enthusiastic SLT to provide services to adults in outpatient, community and inpatient settings. The types of disorders the service supports with include:

- Acquired communication disorders

- Speech and/or language disorders

- Eating and drinking disorders

Main duties of the job

To work independently to a specialist level of expertise and be accountable for own professional actions

To work as an active member of the adult S&LT team in Hertfordshire and to contribute specialist clinical expertise to local networks where appropriate

To determine and carry out appropriate assessments for complex clients and to evaluate the information gained to make appropriate clinical decision based on highly specialist clinical knowledge and including a different diagnosis

To maintain comprehensive and accurate assessment and treatment records for clients in line with the professional standards and local Trust policies

Within a complex caseload to determine and structure appropriate care plans for intervention including treatment, advice, review, discharge, information exchange and communication with client, team members and carers following assessment.
