Job responsibilities
1.
Key Responsibilities
Provide
follow up care for service users who struggle to access care and support through
the traditional 9-5 operating hours as well as offer care and support to other
service user who requires support within 8-8pm.
Work
closely with colleagues in specialist community mental health services to
ensure smooth transitions between teams and services and facilitate an easy
in, easy out approach to improve access to evidence-based interventions.
Support
service users through co-produced assessments and evidence-based interventions,
including the completion of Dialog+ care plans.
Support
with processing referrals, triaging and liaising with colleagues and carers in
the provision of holistic support.
Share
in the clinical management of a defined caseload and develop an interface with
other services and agencies so that service users are offered focussed and
co-ordinated packages of care which are appropriate to their needs.
Alongside
the Team and Service Manager, take a leadership role in the provision of out of
hours support, supporting with the development of internal processes and the monitoring
of the impact of the support provided.
3.1 Financial responsibility
To operate within agreed budgets and ensure
compliance with Standing Financial Instructions and other policies and procedures and
any audit recommendations.
3.2 Responsibility
for Human Resources
To
maintain compliance with the Trusts approach to Human Resources, including the
developing of training plans for the teams.
In
line with the Trusts polices, take the leading role in the investigation of
serious incidents and complaints.
3.3 Responsibility
for Administration
To
produce high level reports to the Division as required.
To
maintain up to date clinical records in a timely and accurate manner in line
with standard operating procedures.
Improve
and maintain statistical records of work data within the services to include
own and other accurate records.
Report
to the Team Manager any serious untoward incidents and ensure documentation and
records are in accordance with Partnership guidelines.
To
demonstrate and awareness of the Partnership policies, procedures, legislation
and practice within the guidelines in particular critical incident procedure,
child and adult safeguarding, risk assessment.
3.4 Responsibility
for IT and Digital Systems/Services
Ensure
systems are in place to collect social care performance activity management data as required.
2.
Clinical Responsibilities
To
work in collaboration with clinical leads and all professionals in the MDT and
clinical leads.
Provides
a brief clinical risk and mental health assessment, formulation and short-term
contingency planning for people presenting in mental health needs or support.
Provide
care or support via telephone/Video or a face-to-face assessments.
Work
closely with other mental health services and agencies; such as the police,
ambulance, local authority and third sector.
Using
a range of brief solution-focused interventions, to empower people, carers and
their families in decision making; diverting to a range of health and social
care resources for support.
Communicate
with service users and callers at their level of understanding, culture and
background. Always remain courteous and professional.
Provide
skilled and detailed triage, full assessment and intervention to clients for
all follow up or new referrals to the team. In the event of support during mental
health crisis, re directing to the most appropriate services.
Offer
support and advise to carers in prevention and management of crisis situation.
Provide
clinical advice, education, support and training to team colleagues, CMHTs,
primary healthcare teams, other professionals, service users and carers on the
management of mental health problems.
Deliver
care that is evidence based, reflecting current best practice and research.
To
maintain safe and acceptable standards of practice in accordance with Trust
policies and procedures.
Participate
in the development, mentorship and clinical supervision of staff and students.
Practice
in a way that minimises dependency and promotes recovery.
Demonstrate
a sound understanding of the legal and ethical issues in the provision of
mental health support.
In
particular, demonstrating sound understanding of the Mental Capacity Act (2005)
and the Mental Health Act (2007) and its use in a non-mental health setting.
Provide
clinical advice and support to other professionals about risk management and
mental health via the professionals' line.
To
keep professionally updated with ongoing developments in mental health though
an evidence-based approach to delivering high quality mental health services by
evaluating, updating, imitating and/or participating in training, audit and
research.
The
post holder will be expected to adopt a flexible attitude to take account of
the changing needs of the service as dictated by the service user and
organisational lead.
Continue
the promotion of a culture of evidence-based practice and support specialist
practitioners in their role
Reducing
and managing risks to service users through collaborative working together with
service users/carers in their treatments and adherence to their care-plans.
5. Operational Responsibilities
Deputise
in the absence of the Team Manager
The
post holder will be required to undertake any other duties and responsibilities
within the framework of the post as determined by the Managing Director.
This post is subject to an annual review of performance to agreed
targets. Targets will be agreed with the
Managing Director and the post-holder and will be subject to regular review of
progress.
6. Policy / Service Development
Comply
with statutory social care guidance and partnership care policies.
7. RESEARCH AND DEVELOPMENT
Promote
and encourage research and evidence-based care, including participating and
collaborating in research and QI projects.
Undertake
clinical and others audits as directed by your manager.
Evaluate service and performance using a
range of information sources to identify and implement service improvements.
