Ward/Dept: DHTA QVM

Hours: Full time Up to 37.5 hours per week

Shift Pattern: Mon - Fri, 8am-6pm

Pay Rates Day: £22.89

At Hertfordshire Community NHS Trust, they are seeking a passionate Band 6 Occupational Therapist to bring their expertise to its well-established community therapy team across a number of localities working within East and North Hertfordshire, to help continue to provide excellent rehabilitation to patients within their home environment. You will have the opportunity to join the discharge home to assess service. Discharge home to assess, assesses patient within the first 72 hours of discharge from hospital and provide up to 21 days rehabilitation beds.

Support the Discharge Home to Assess Manager in delivering of high standard therapy service, including service evaluation.

Undertake therapeutic assessment of patients, some of whom may have complex and diverse conditions, and delegate tasks as appropriate.
