Job Title: Physiotherapist/Occupational Therapist
Grade: Band 6 - would consider band 5 if competent and does not require supervisory type of support

Hours: Full time 37.5 hours per week (would consider part time hours)
Shift Pattern: Monday -Friday, 8am 4pm
Pay Rates Day: £21.80
The Role
We are looking for a Physiotherapist OR Occupational Therapist to join our forward thinking and innovative team to continue to provide excellent rehabilitation to patients within their home environment. Discharge home to assess, assesses patient within the first 72 hours of discharge from hospital and provide up to 21 days rehabilitation.

The therapist will work alongside physiotherapists, therapy assistants, social workers, community nurses, GPs, and carers. Staff meet regularly for professional development and peer support; the post holder will have the opportunity to supervise junior staff and offer placements to students.
This post is full time but applicants who wish to work part time or flexibly will also be considered.
To support the Discharge Home to Assess Team Leads in delivering of high standard therapy service, including service evaluation.
To undertake therapeutic assessment of patients, some of whom may have complex and diverse conditions, and delegate tasks as appropriate.
Following assessment to provide, develop and implement individual care/treatment plans.
To be part of an integrated health and social care team. To provide clinical leadership and support to the multidisciplinary team in providing care to avoid inappropriate admissions to hospital and support hospital discharge.
To provide assessments, and treatment, where required for vulnerable patients with potentially complex needs. To support patient-led goal working within the community.
