Main duties of the job
The post holder:
Will
assist the Lead Pharmacist in the provision and development of clinical
pharmacy services to the wards on the Watford site and across the trust.
Will provide a patient
focused clinical pharmacy service, assuming responsibility for meeting the
therapeutic goals of both patients and prescribers.
Will monitor
prescriptions for safe, economic, effective and evidence based prescribing
enforcing and actively promoting compliance with the Formulary.
Will provide complex
medicines information to patients, carers, doctors, nurses and other
professionals including information on side effects, dosing and indications
which is appropriate and clear for the recipient.
Will assist with the
development and implementation of systems for the extended use of patients own
drugs.
Skills/Responsibilities:

Registered Pharmacist (GPhC)
MPharm degree or equivalent qualification
Experience within NHS pharmacy practice
System Knowledge: Ascribe, EMIS, Cerner EPR
Ward cover experience in hospital setting
Effective communication and telephone skills

In addition to experience, all Qualified roles will be required to

a. Clinical Duties
To assist the Lead Pharmacist in the provision and
development of clinical pharmacy services to the wards on the Watford site and
across the trust.
To provide a patient focused clinical pharmacy service,
assuming responsibility for meeting the therapeutic goals of both patients and
prescribers.
To monitor prescriptions for safe, economic, effective and
evidence based prescribing enforcing and actively promoting compliance with the
Formulary.
To provide complex
medicines information to patients, carers, doctors, nurses and other
professionals including information on side effects, dosing and indications
which is appropriate and clear for the recipient.
To assist with the development and implementation of
systems for the extended use of patients own drugs.
To contribute to
the ongoing development of technician led systems for ensuring the safe,
prompt, accurate and seamless provision of medicines to patients.
To provide
professional advice on pharmaceutical matters to medical, nursing and other
professional staff.
To communicate information about medicines, treatment
goals and follow up to community pharmacists, GPs and other members of the
primary healthcare team as appropriate.
To monitor for adverse drug reactions and submit yellow
cards to the MHRA.
To participate in the delivery of dispensary services for
inpatients and outpatients liaising with the Dispensary manager to review and
monitor workflow and ensuring legal, national and local standards are complied
with and appropriate records completed and maintained.
b. Management and Supervision
To provide day to day supervision of the work of
Pre-registration Pharmacists, Band 6 Pharmacists, Clinical Technicians,
Assistants and undergraduate Pharmacy students attached to the area of rotation
and within the Pharmacy Department.
To promote the efficient use of Pharmacy staff in the area
of rotation and within the Pharmacy department.
To participate in departmental Clinical Governance
meetings
c. Training and Education
To act as a role model and assist with training of
pre-registration pharmacists, pharmacists, pharmacy technicians and assistants.
To maintain up to date knowledge of training
opportunities/courses and help match these to training needs for individuals.
To actively participate in education and training programs
and to keep up to date with developments in Pharmacy.
To promote and facilitate Continuing Professional
Development (CPD) within the Pharmacy Department.
To assist with the training of medical, nursing and other
healthcare staff.
d. Other Duties
To achieve
objectives agreed at individual performance review.
To participate in
writing guidelines relating to the use of medicines.
To contribute to the preparation and update of protocols.
To assist with providing advice, information and the
preparation of reports for healthcare committees if nominated to do so.
To co-operate with Medicines Information staff in
responding to information requests arising from the Clinical Services.
To record Clinical Pharmacy Interventions in line with
departmental policy.
To report any drug errors detected to your line manager
and ensure that the appropriate incident report form is completed.
To undertake controlled drug checks regularly in accordance
with trust policy
To carry out audit projects to assess prescribing practice
and adherence to guidelines with the aim of improving patient care.
To identify opportunities for pharmacy practice research.
To participate in other aspects of delivery of the
