Main duties of the job
What you will be responsible for
Take
the lead and assist other staff in assessing service users needs, planning,
implementing and evaluating care plans and interventions designed to meet
service users individual needs (in accordance with the model in use in the
particular service area) and enable them to exercise choice over their own
lives.
Actively
involve service users in developing and agreeing their own care plans and to
act as an advocate for users of the service ensuring that there is proper
regard for their dignity, choice, self- esteem, racial, cultural, sexual and
religious needs and preferences.
Ensure
that risks are managed effectively and that an appropriate balance is achieved
between risk and the benefit to the service user.
act
as the Named Nurse and take responsibility for planning and providing care to a
defined group of service users within the service and for communicating with
other professionals, agencies, relatives and carers where that is appropriate
and having due regard for issues of confidentiality.
The role will involve, but not be limited to you performing the
above.
