Job Title: Community Psychiatric Nurse
Trust Location:Greater Manchester Mental Health NHS Foundation Trust
Pay Rates:£18-£29 Per Hour

(GMMH) provides inpatient and community-based mental health care for Bolton, Manchester, Salford, Trafford
and Wigan. The Trust also provides a wide range of specialist mental health and substance misuse services for

What you'll be responsible for:
- Providing robust assessment of patients (by telephone or face-to-face) who have been
referred to the CMHT due to presenting with mental health difficulties.
- Identifying appropriate care pathways and ensuring the least restrictive option to meet the
needs of the patient and ensuring their wellbeing.
- Signposting to appropriate care providers (including primary and tertiary care). Attending daily
Red Board meetings.
- Raising concerns to senior staff for support and management. Completing RiO documentation
and planning care for patients.
- Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
- Liaison with colleagues nursing, occupational therapy, medics, psychology and admin.

- A minimum of 6 months paid experiences a Community Psychiatric Nurse in an UK hospital environment within the last 3years.
- Comply with NMC Professional Registration and keep up to date with current nursing practices.
