Job summary
Tendring PCN has an exciting opportunity for a social prescriber to join our PCN on a permanent basis.

Main duties of the job
- Receiving and managing all referrals from multi-agencies, utilising various referral platforms.
- Providing personalised support to vulnerable individuals.
- Working in partnership with the practice team to support patients and promote positive lifestyle changes, with the aim of preventing ill-health by means of behavioural change techniques.
- Completing holistic care planning assessments, analysing how needs can be met by services and other opportunities available in the community.
- Providing information and advice about a wide range of services in order to promote health and well-being in order to support independence within communities.
- Forging strong links with local VCSE organisations, community and neighbourhood level groups.
- Contributing to reducing demand on statutory services, hospital admissions and to combat unnecessary GP appointments.
- Working with patients in the practice, or with patients in their own homes, to provide personalised person-centred support.
- Be part of a proactive, preventative approach in Primary Care
- Help reduce health inequalities and strengthen community connections
- Keep accurate records in the clinical systems.
About us
Our network is located on the North East Essex Coast and comprises of 3 GP practices: Mayflower Medical Centre, St James Surgery & Great Bentley Surgery. It has an approximate patient population of 50,000 and it strives on bringing together local health care and resources to deliver high quality, evidence-based and integrated care to all their patients.
The successful candidate will work as part of a multidisciplinary team, contributing alongside the practice. They will support and be responsible for areas of medicines optimisation and delivery of excellent management of medications to ensure high quality, safe and efficient patient care in general practice.
