Care Coordinator

Job summary

Ludham and Stalham Green Surgeries are looking to welcome a care coordinator to their friendly team on a part-time basis, working approximately 17 hours per week. The agreed hours would need to be worked on a Monday and a Thursday each week. Please note this is a practice/PCN role, so you may be required to work at other NN4 practices. This role would be key in providing and maintaining a high standard of care for patients health needs. The post holder will be an integral part of the general practice team.

Main duties of the job

  • Oversee and coordinate patient requests via the digital Accurx system.
  • Identifypatients who need to be referredback to other health professionals within the PCN.
  • Providecoordination and navigation for people and theircarersacross health & care services.
  • Support patients to utilise decision aids, help create single personalised care & support plans.
  • Help people to manage their needs through answering queries, making & managing appointments.
  • Ensure that people have good quality written or verbal information to help them make choices about their care.
  • Assist people to access self-management education courses, peer support or interventions that support them in their health & wellbeing.
  • Provide the interface between primary care & other services, helping patients with the transition between services.
  • Attend and participate in any practice meetings when required.
