Mendlesham Medical Grouphas a fantastic opportunity foran experienced or a newly qualified practice nurse to join our team on apart-time basis,for 18 hours per week on Thursdays and Fridays. The requirement is for approximately 6-12 months.Whether youre looking for full-time or part-time work, wed love to discuss how you can fit into our thriving practice.
Main duties of the job
- Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
- General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
- Managing and treating long-term conditions, in line with national and local policies and the practice needs.
- Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
- Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
- Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
- Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
- Meeting the needs of patients presenting for opportunistic wound care.
- Hypertension review and management.
- Running asthma, COPD and diabetes clinics and reviews.
