Job summary

Due to retirement, Mount Farm Surgery has a fantastic opportunity for an experienced nurse manager to join our team on a full or part time basis - there is some flexibility in hours and working pattern for the right candidate.

Main duties of the job

  • Line management of the nursing team, conducting regular one-to-ones and appraisals in line with practice policy
  • Lead long-term condition management within the organisation, supporting quality improvement and assurance initiatives.
  • Run asthma, COPD and diabetes clinics and reviews.
  • Support public health and screening programmes, including immunisations, vaccinations and cervical screening
  • Embed population health management approaches within general practice, targeting health inequalities and improving access
  • As required, provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
  • Support, mentor and supervise new-to-practice nurses and other healthcare professionals within the organisation
  • Prescribe medication for patients in accordance with PGDs and own scope of practice
  • Participate in clinical audits and research projects and implement changes as required
  • Work within national and local protocols where these exist
  • Recognise the boundaries of their practice and know when and to whom patients should be referred
  • Demonstrate initiative and be creative in finding solutions to problems
  • Support in the delivery of enhanced services and other service requirements
  • Contribute to practice targets (QOF etc.), complying with relevant guidance
  • Maintain accurate clinical records
  • Maintain chronic disease registers and implement an effective call/recall system

About us

Mount Farm Surgery is a modern, efficient, and well-equipped GP surgery in the Moreton Hall area of Bury St. Edmunds. First established in Looms Lane in Bury St. Edmunds, we moved to the Moreton Hall Estate approximately thirty years ago, which was built on the original Moreton Hall farmland at the top of Mount Road (hence the name Mount Farm). The practice currently has just over 16,000 registered patients.

Proactive and holistic care:We endeavour to offer proactive care to ensure the best of health for our patients. Often health issues are related to other aspects of a patients lifestyle, and we believe in helping patients improve their overall health and well-being, wherever possible taking the opportunity to prevent illness and not just treat it. We routinely check up on patients with long-term conditions to ensure we are providing effective care. We also run a series of clinics and patient workshops that provide valuable help and support to those dealing with long-term conditions such as asthma and diabetes.

A GP training practice:Some of our GPs have the additional skills, experience, and qualifications to support qualified registrars through their vocational training to become a GP. This also ensures that our own services are in line with the latest medical knowledge and practices.

  • Evidence of clinical skills and experience in treatment room consultation, phlebotomy, ECG recording, screening, management of long-term conditions, cervical cytology sampling, vaccinations and immunisations, assisting with minor surgery, dressing and wound care.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
