Care Co-ordinator

Job summary

Colchester Medical Practice has an opportunity for a care co-ordinator to join our team on a full-time basis working 37 hours per week over 5 days. This role is based at our Wimpole Road Site.

The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients.

Main duties of the job

  • Working within our proactive care team, following up with patients who have recently been discharged from hospital.
  • Working with GPs and other professionals within the practice to identify and manage a caseload of patients. Particularly working with vulnerable patient groups with long-term conditions that may require this extra support. This may include working with individuals, their carers, families and healthcare teams.
  • Using in-depth knowledge of locally available support and services todevelop care plans with patient, focusing on prevention.
  • Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
  • Helping patients prepare for, or follow-up after, clinical conversations.
  • Supporting digital initiatives.
  • Using clinical systems on a daily basis, with confidence to run searches and keep care records up-to-date.
  • Dealing with day to day admin re safeguarding.
