PCN Support Care Co-ordinator - Contract position

Job summary

North Uttlesford Primary Care Network are seeking a proactive and detail-oriented individual to support our PCN team in delivering safe, effective, and coordinated care to patients. This is a full-time, fixed term contract for 9 months that will require you to work from different locations including GP practices, from home or at PCN offices so flexibility is essential. Appropriate equipment and support will be provided to facilitate remote working where applicable.

This role involves identifying patients for review, organising investigations, supporting appointment booking, and coordinating care across the wider team. The post holder will play a vital role in improving patient access and helping to meet Quality and Outcomes Framework (QOF) and Enhanced Care Framework (ECF) targets. It involves working closely with the teams comprising the PCN, the GP admin team, and wider multidisciplinary teams to ensure coordinated, patient-centred care.

You may be asked to adapt your role and responsibilities depending on the needs of the service, including taking on additional tasks or changing priorities as clinical demands evolve.

Training and support will be provided as needed and this role may develop over time; the post holder will be expected to contribute to service improvement and innovation.

Main duties of the job

  • Patient identification and appointment planning
  • Run clinical system searches to identify patients eligible for ARRS clinic appointments.
  • Check patient records for upcoming or overdue blood tests and outstanding investigations.
  • Coordinate care by checking for existing appointments or planned interventions and avoid duplication.
  • Investigations and Test Coordination
  • Organise and request blood tests using T-Quest and book into the appropriate clinics.
  • Ask patients to provide home blood pressure (BP) readings and offer clear guidance on how to do this correctly.
  • Use both telephone and text messaging service to contact patients and carers.
  • Consider special circumstances such as housebound patients and make referrals to the Community Care Coordination team when appropriate.
  • Clinical system checks and referrals
  • Identify QOF and ECF indicators in patient records for key clinical areas which may be addressed during reviews.
  • Patient communication and support
  • Book patients into ARRS clinics following results, consider whether a single or double appointment is needed, based on complexity.
  • Annotate the appointment slot with key information, including what is required (e.g. medication review, results discussion, follow-up action.
  • Consider the most appropriate consultation type based on the patient's clinical needs, mobility, and social circumstances.
  • Support practices with administrative changes and implementation of new programmes of work, eg new digital tools.

North Uttlesford Primary Care Network consists of four GP practices providing care to a rural population of approximately 43,000 patients. We are seeking a proactive and detail-oriented individual to support our PCN team in delivering safe, effective, and coordinated care to patients. The post holder will play a vital role in identifying patients for review, organising investigations, supporting appointment booking, and coordinating care across the wider team with the aim of improving patient access and care.
