Practice Nurse

Job summary

Grove Medical Centre in Felixstowe are looking for an experienced practice nurse to join their busy team on a full-time basis. As a key member of our practice, you will be able to treat patients along with the support of our team of GPs, nurse practitioners, practice nurses, health care assistants, and dispensary and administration teams.

Main duties of the job

• Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
• Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
• General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
• Managing and treating long-term conditions, in line with national and local policies and the practice needs.
• Reviewing medication and working with patients to support their adherence to prescribed treatments.
• Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
• Meeting the needs of patients presenting for opportunistic wound care.
• New patient and health checks.
