The closing date is 31 October 2025
Job summary
The SS9 integrated neighbourhood team is seeking a HCA to join us on a part-time basis for 15 hours per week, or full-time (37.5 hours per week) when combined with the integrated neighbourhood care-coordinator role, which we are also advertising.
The post-holder will provide additional time, capacity, and expertise to our integrated neighbourhood team, helping us to enhance the care we provide to our patients. - Deliver health promotion, lifestyle advice, and education to empower patients in self-management.
- Accurately document patient information in electronic health records and communicate updates to the wider care team.
- Support and maintain infection prevention and control practices within community and home settings.
- Support the INT with administrative duties.
- Ensure all delegated clinical tasks are carried out safely, effectively, and within scope of competence.
- Contribute to the collection of data and audit activities to monitor service outcomes.
- Work in line with agreed protocols, clinical guidelines, and quality standards.
- Support the INT in achieving national and local performance targets.
- Promote patient safety by reporting incidents, near-misses, and safeguarding concerns promptly.
