Healthcare Assistant - Integrated Neighbourhood Team

Job summary

The SS9 integrated neighbourhood team is seeking a HCA to join us on a part-time basis for 15 hours per week, or full-time (37.5 hours per week) when combined with the integrated neighbourhood care-coordinator role, which we are also advertising.

The post-holder will provide additional time, capacity, and expertise to our integrated neighbourhood team, helping us to enhance the care we provide to our patients. The healthcare assistant role within general practice can be very different to the role within other settings and as such we are only looking for applications from those with previous primary care experience. All applicants should ensure that they live within reasonable commuting distance.

Main duties of the job

  • Support the Integrated Neighbourhood Team (INT) in delivering high-quality, holistic patient care within the Primary Care Networks (PCN).
  • Carry out clinical observations (e.g., blood pressure, pulse, temperature, oxygen saturation).
  • Perform basic diagnostic tests (e.g., urine dipstick, blood glucose monitoring).
  • Support with wound care, dressings, and removal of sutures/clips (where trained).
  • Ensure work is carried out in line with core competencies.
  • Contribute to the safe transition of patients back into the community, supporting recovery and reducing hospital readmissions.
  • Provide a patient-centred approach in the home setting, ensuring dignity, safety, and confidentiality at all times.
  • Deliver health promotion, lifestyle advice, and education to empower patients in self-management.
  • Accurately document patient information in electronic health records and communicate updates to the wider care team.
  • Support and maintain infection prevention and control practices within community and home settings.
  • Support the INT with administrative duties.
  • Ensure all delegated clinical tasks are carried out safely, effectively, and within scope of competence.
  • Contribute to the collection of data and audit activities to monitor service outcomes.
  • Work in line with agreed protocols, clinical guidelines, and quality standards.
  • Support the INT in achieving national and local performance targets.
  • Promote patient safety by reporting incidents, near-misses, and safeguarding concerns promptly.

About us

The Integrated Neighbourhood Team (INT) operates across both SS9 North and SS9 South Primary Care Networks (PCNs), providing essential support to patients who are housebound or transitioning from hospital care back into the community. The teams primary aim is to deliver person-centred care in a timely and coordinated manner, helping individuals remain safely at home, optimise their independence, and reduce the likelihood of unnecessary hospital readmissions.

The INT is a multidisciplinary team made up of professionals from Primary Care, Secondary Care, Tertiary Care, Social Care, and the Voluntary Sector. This includes GPs, nurses, emergency care practitioners, social prescribers, and allied health professionals. By working collaboratively, the team delivers integrated care, welfare checks, and personalised support tailored to each patients unique needs.
