Social Prescriber/Care Coordinator

Job summary

Ambrose Avenue Group Practice is seeking a motivated and compassionate social prescriber/care coordinator to join our growing team on a full-time basis. This is an exciting opportunity to play a vital role in supporting patients by ensuring they receive the right care at the right time from the appropriate services.

As a social prescriber/care coordinator, you will be an integral part of our multi-disciplinary team (MDT) and will work closely with our Primary Care Network (PCN) team. The successful candidate will collaborate with other members of the MDT as needed, connecting patients to community groups and statutory services for both practical and emotional support.

Main duties of the job

  • Collaborate with GPs, nurses, and other healthcare professionals to support patients with complex health and social care needs.
  • Assist patients in navigating the health and care system, ensuring they are connected with the most suitable services.
  • Provide personalized support to vulnerable individuals and their caregivers.
  • Coordinate care for people with long-term conditions, frailty, and those requiring proactive management.
  • Work collaboratively with local organizations to promote positive lifestyle changes.
  • Facilitate meetings with organizations to manage and drive new initiatives.
  • Offer information and advice regarding a wide range of services to help promote independence within communities.
  • Establish strong connections with local organizations, community, and neighbourhood groups.
  • Contribute to reducing the demand on statutory services.
  • Work with patients in the practice or in their own homes, providing personalized, person-cantered support, including the use of care technology.
  • Help improve patient experience and outcomes within the PCN.
