Proactive Care Lead

Job summary

Are you a passionate and experienced advanced nurse practitioner or advanced paramedic ready to lead innovative, preventative care in the heart of our community? Clacton PCN is looking for a proactive care lead to drive forward integrated, person-centred care across our network.

As a proactive care lead, you will head up a dynamic multidisciplinary team including nurses, care coordinators, paramedics, HCAs, and social prescribing link workers.

You'll work closely with PCN leadership and wider system partners to deliver care that empowers patients, reduces hospital admissions, and supports people to live well at home for longer.

This role is subject to a 6 month probation period.

Main duties of the job

You'll be responsible for delivering high-quality, proactive, and preventative care with a particular focus on:

  • Frailty management
  • Cardiovascular disease prevention
  • Enhanced Health in Care Homes (EHCH)
  • Embedding population health management approaches

  • Independent prescriber (V300)
  • Master's level qualification in Advanced Practice or Leadership

Experience

  • Full UK driving licence
