Job summary

SS9 South PCN has an opportunity for a care coordinator/healthcare assistant to join our team on a full-time basis for 37.5 hours per week. The role will be based across Pall Mall and Highlands Surgeries in Leigh on Sea.

The post-holder will provide extra time, capacity, and expertise to our practice teams, helping us improve the care we provide to our patients.

Main duties of the job

  • Support SS9-South PCN in the delivery of high-quality primary health care projects and systems.
  • Support the management teams and clinical director to ensure the smooth running of the PCN operations.
  • Coordinate attendance for the SS9-South PCN in meetings with external stakeholders, ensuring appropriate personnel attend in relation to PCN services.
  • Coordinate the deliverance of seasonal covid vaccination programmes including ordering, arranging mutual aid, and reporting to the ICB.
  • Oversee the deliverance of the Enhanced Access programme, to include setting rotas, sourcing and agreeing staff allocations. Overseeing the usage of appointments and undertaking any required reporting to the ICB.
  • Support rota and allocations for the ECP teams and provide daily support for allocation of visits and route planning.
  • Assist in HR provision ensuring appraisals/1-2-1s and Return to Work meetings are delivered timeously.
  • Arrange and coordinate mandatory training for clinicians and staff within the SS9-South PCN including practices own staff as well as PCN employed staff.
  • Use of IT systems including clinical and administrative to support smooth running of the SS9 South PCN including updating holiday requests, appraisals and policies.
  • Support the SS9 South PCN and associated teams with deliverance of services to housebound patients.
  • Provision of HCA support to housebound patients including phlebotomy, blood pressures, diabetic foot checks, within own range of competencies.

About us

Our PCN is known as the SS9 South PCN and comprises a group of practices, Highlands and Pall Mall Surgeries, supporting 36,000 patients. Additional healthcare professionals are being recruited to work on behalf of Primary Care Networks.

New roles include advanced nurse practitioners, clinical pharmacists, mental health professionals and social prescribing link workers. By introducing a greater skill mix, local people will get more access to the support they need to keep people healthy and independent.

Experience

  • Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups.
