Job summary
Tiptree Medical Centre are a well-established, patient-centred GP surgery looking for an enthusiastic and experienced general practice nurse, who ideally also has specialist skills in chronic disease management such as diabetes and asthma/COPD. This role can be flexible to hours worked, so please apply to discuss further.
This is a fantastic opportunity for a proactive nurse who is passionate about improving long-term condition management and delivering high-quality, personalised care within a supportive primary care setting.
Main duties of the job
- Provide holistic general nursing care, including immunisations, wound care, cervical screening, and long-term condition management.
- Lead and develop diabetes clinics, supporting patients in managing their condition through lifestyle, medication, and monitoring.
- Work closely with GPs, healthcare assistants, and external services to ensure continuity of care.
- Support QOF and enhanced services achievement.
- Contribute to health promotion, audits, and quality improvement initiatives within the practice.
