First Contact Practitioner/Physiotherapist

Job summary

Stanford Le-Hope and Corringham PCN has an opportunity for an MSK FCP physiotherapist to join our primary care network (PCN), to support our 6 GP practices across Stanford Le-Hope and it's surrounding areas, to assist in re-shaping primary care across our communities. This role is offered on a full-time basis or part-time basis, for 22.5 - 37.5 hours per week, on 3 to 5 day basis. We can be flexible with how many days and what days you are wanting to work.

We aim to offer a single point of access for clinical assessment and treatment, delivered from our PCN estate in Corringham. The successful candidate will benefit from this unique opportunity to develop effective communication and strong working relationships across our organisation and will be actively involved in how their role evolves over time.

Main duties of the job

  • Deliver a high-quality first contact physiotherapy (FCP) service to patients registered with SLH PCN, operating from within PCN premises.
  • Serve as the first point of clinical contact for patients, providing expert physiotherapy assessment and decision-making regarding the most appropriate course of care, including for undifferentiated conditions. This will involve consulting directly with patientsoften without prior GP involvement - to establish timely, accurate diagnoses and tailored management plans for musculoskeletal (MSK) conditions.
  • Request, follow up on, and interpret appropriate diagnostic investigations as needed to support diagnosis and treatment planning. This includes a sound understanding of how to interpret results from investigations such as x-rays and relevant blood tests, recognising their limitations, sensitivity, and specificity.
  • Support the implementation of self-management programmes and provide holistic, patient-centred care. This involves empowering patients to manage their conditions where appropriate, promoting healthy lifestyle choices, and enhancing their physical activity, mobility, independence, and quality of life, while aiming to reduce reliance on pharmacological treatments.

And serving over 30,000 patients. We have a team of 16 affable helpful staff who mainly all work out of Corringham health centre.
