Job summary

Riverside Health Centre has an exciting opportunity for a clinical practitioner to join our practice in a new role to help us continue to offer the best possible service to our increasing patient population. This position is offered on a part-time basis for 22 - 35 hours a week. This role will mainly be based in the surgery, but some remote working may be required.

This is a mixed role working closely with the GPs and wider team and involves clinical triage, face to face and telephone consultations, home visits and proactive care for our local care home residents.

Clinically they will be responsible for receiving and managing patients with undifferentiated illness to assess their healthcare needs and then diagnose and provide treatment in line with local and national guidelines to ensure a high-quality service to our population.

Main duties of the job

  • Diagnosing and treating patients presenting to primary care with acute medical conditions, minor illness and mental health problems. This may be in the surgery, remote telephone/online consulting or as a home visit.
  • Working with the GPs to provide excellent proactive care for care home residents including regular ward round reviews.
  • Communicating with patients/parents/carers including breaking bad news and providing appropriate ongoing support to meet identified needs.
  • Supporting the duty doctor with clinical triage.
  • Ensuring clinical practice is safe and effective and working within boundaries of competence.
  • Initiating and arranging appropriate investigations and referrals to other members of the primary and secondary health care teams as clinically indicated.
  • Participating in the administrative and professional responsibilities of the practice team including clinical audit.
  • Ensuring the clinical record is completed and maintained with accurate and contemporaneous details recorded.
  • Participating in the education and training of students of all disciplines and the induction of all members of the practice staff where appropriate.
