Job summary

Tendring PCN has a fantastic opportunity for experienced practice nurses to join our team on a full-time basis for 37.5 hours per week.Salary, working patterns and locations are to be discussed at interview. These positions are offered on a 18 month fixed term contract.

Our PCN covers patient populations registered at Great Bentley Surgery in Great Bentley, St James Surgery in Clacton and Mayflower Medical Centre in Harwich.

General nursing applications are welcomed, with areas of special interest and experience desirable including:

  • Respiratory conditions
  • Diabetes management
  • Minor illness
  • Family Planning - coils and implants
  • HRT, menopause, women's' health

Main duties of the job

  • Undertaking health assessments, administering medication and treatments, and performing diagnostic tests.
  • Providing and evaluating a range of health interventions to ensure that patients health and social care needs are met.
  • General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition.
  • Managing and treating long-term conditions, in line with national and local policies and the practice needs.
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Supporting and managing the health needs of women presenting for sexual health and contraception, and cervical cytology consultations.
  • Implementing, and participating in, vaccination and immunisation programmes for both adults and children, including patients travelling abroad.
  • Meeting the needs of patients presenting for opportunistic wound care.
  • Hypertension review and management.
  • Running asthma, COPD and diabetes clinics and reviews.
  • New patient and health checks.

Tendring Primary Care Network (PCN) is a collective group of General Practices within Tendring, North East Essex:

  • Great Bentley Surgery, Great Bentley (GBS)
  • St James Surgery, Clacton (SJS)
  • Mayflower Medical Centre, Harwich (MMC)

Our aim is to deliver better health outcomes for our local patient populations, and reduce health inequalities, via provision of additional roles and services to extend the general practice teams.

Tendring PCN are one of the 22 PCN test sites as part of the national PCN pilot programme, a partnership with SNEE ICB and NHS England, to narrow demand and capacity gaps, improve workforce and patient experiences, as well as sustain general practice for the future. Fixed term contract extensions are subject to continued funding.

  • Fundamentals of Practice Nursing course desirable (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.

  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.

  • Insight and understanding of current issues in nursing and the wider NHS.
