Health and Wellbeing Coach

The closing date is 27 June 2025

Job summary

EAST:MK Primary Care Network has an exciting opportunity for a health and wellbeing coach to join our practice team, on a full-time basis, for 37.5 hours per week. You will be working over 5 days, between Monday and Saturday, on a rotation basis across our 4 surgeries; Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice and The Grove Surgery, that make up the PCN.

You will work collaboratively with the multidisciplinary team (MDT) within the PCN, ensuring all patients receive the best possible care and service. The focus of the role is to support our patients to feel in control of their life and make the necessary behavioural changes to work towards their personal goals.

You will be coaching and motivating patients through multiple sessions, to identify their needs, set goals and support them to implement their personalised health and care plan to ensure that they are active participants in their own healthcare, empowering them to take more control in managing their own health and well-being, to live independently and to improve their health outcomes.

Main duties of the job

  • Care-planning and the delivery of systematic self-management support, based on the knowledge of everyone's activation levels.
  • Care-coordination across the care continuum (including identification and support of carers).
  • Supporting effective team-working in primary care, through taking on appropriate practice-based tasks, attending regular team meetings, if required, and working with the complex-care teams.
  • Delivering group and one-to-one sessions on healthy eating, physical activity and health and wellbeing, to tie-in with personalised care plans.
  • Engaging patients, and using health-coaching techniques, to support them to take an active role in their health and wellbeing.
  • Supporting practices where necessary, including NHS Health Checks, Smoking Cessation, and outreach programmes.
  • Helping to raise the local populations awareness of the support groups and the opportunities available to assist them in achieving their health and wellbeing goals.
  • Working in partnership with clinical and non-clinical colleagues, management support and the wider PCN, to ensure the role delivers the best possible outcomes for our patients.
