Job summary

The Colte Partnership is currently looking for a social prescriber to join our team for 25 hours per week. The social prescriber will play a vital role in enhancing patient care and well-being by connecting individuals with non-medical support services within the community.

There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. You will be joining an enthusiastic team of clinicians and administrators. You will be well supported with on-going professional development (training support, CPD and peer support). The role will contribute to the improving quality of care of our patients across the network of practices.

Main duties of the job

  • Collaborate with healthcare professionals to identify patients who would benefit from social prescribing.
  • Conduct holistic assessments to understand patients' social needs and preferences.
  • Develop personalised care plans and signpost patients to relevant community resources and services.
  • Educate patients on available social support options and empower them to take control of their health.
  • Organise and facilitate group activities that promote mental well-being, physical health, and social inclusion.
  • Maintain accurate records of patient interactions and outcomes in accordance with data protection regulations.

About us

The Colte Partnership is a GP super-partnership, formed in June 2017. It brought together 8 local GP surgeries across Colchester and Tendring, serving approx. 80,000 patients. With the combined workforce, skills and knowledge, we are in a unique position to assist our patients across the county.

As well as General Practice, we offer Extended Access appointments to enable all of our patients to access medical care at evening and weekends; we work collaboratively with external agencies to provide in home services and Social Prescribing; we are working to be at the forefront of Clinical Research; and have many more exciting ventures on the horizon.

We also work as a collaborative Primary Care Network which spans across Colchester and Tendring:

  • Riverside Medical Centre & Ardleigh Surgery
  • Mersea Island Medical Practice, Tiptree Medical Centre & Rowhedge Medical Centre
  • Walton Medical Centre, Colne Medical Centre & Wivenhoe Medical Centre.
