Job summary

Benfleet PCN has an exciting opportunity for a hardworking and enthusiastic emergency care practitioner (ECP)/paramedic, to join our PCN to support our 7 GP practices and assist in re-shaping primary care across our communities. This position is offered on a full-time basis, 37.5 hours per week.

You will undertake home visits for housebound patients, complete ward rounds for our care homes and complete acute visits for our care home patients. You will also be involved in our PACT (PCN Aligned Community Team) model completing complex frailty reviews and advanced care planning (PEACE and DNACPR documents).

ECPs/paramedics work independently within their scope of practice in the community, using their enhanced clinical assessment and treatment skills to assess and manage patients presenting with acute presentations which include minor illness or injury. Main duties of the job

  • Work as part of a multi-disciplinary team (MDT) within the PCN.
  • Provide an alternative model to urgent and same day GP home visit for the network.
  • Assess and triage patients, perform specialist health checks and reviews.
  • Advise patients on general healthcare and promote self-management where appropriate.
  • Perform investigatory procedures as required.
  • Perform and interpret ECGs.
  • Communicate effectively with patients and, where appropriate, family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings and treatment choices.
  • Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required.

Benfleet PCN was formed in June 2019 with seven like-minded practices wanting to provide a joined up integrated service for our population. The PCN now includes over 40 staff members, and we have been known as the accelerator PCN in Mid and South Essex. The PCN provides services for just under 48,000 patients across the patch.

  • Be a registered paramedic, with current HCPC registration.
  • Educated to degree level in paramedicine or equivalent experience.
  • Working towards developing Level 7 capability in paramedic areas of practice and, within six months of the commencement of reimbursement for that individual (or a longer time period as agreed with the commissioner).
  • Completed and been signed off formally within the clinical pillar competencies of the Advanced Clinical Practice Framework.

  • Completed a two-year Consolidation of Learning period when a newly qualified paramedic.
  • Completed a further three years experience as a band 6 (or equivalent) paramedic.
