Job summary

Chet Valley Medical Practice located in Loddon, are pleased to be recruiting a pharmacy technician to join our experienced in-house team on a part-time basis for 30 hours per week. The successful individual will have daily access to an experienced clinical pharmacist plus regular support and supervision sessions.

Main duties of the job

  • Undertake medicines reconciliation in accordance with practice protocols and to update patients medical records/prescribing accordingly.
  • Carry out medicines optimization tasks including effective medicine administration and technical medication reviews.
  • Utilize consultation skills to work in partnership with patients to ensure they use their medicines effectively.
  • Maintain full and complete records of all patient contacts using appropriate clinical templates and coding.
  • Support medication reviews and medicines reconciliation for new care home patients.
  • Support the achievement of the practices prescribing Quality and Outcomes Framework targets.
  • Working with the dispensary manager/ clinical pharmacist, contribute to the review and development of repeat prescribing protocols /standard operating procedures to improve the efficiency and effectiveness of repeat prescribing systems.

About us

Chet Valley Medical Practice is based within the village of Loddon in our spacious purpose-built premises.

We have a list size of 9000 patients and cover an area including Loddon, Chedgrave and the surrounding villages including Norton Sub Course, Hoverston and Surlingham.

Chet Valley is a dispensing practice and have a full nursing team who deal with the long-term condition management as well as two nurse practitioners, an outreach nurse and a paramedic who deals with most of our home visits.
