Barrack Lane Medical Centre has an opportunity for a care navigator to join our friendly, supportive and professional team for approximately 30 hours per week over three days. Although you will be employed by the Barrack Lane & Ivry Street PCN you will be based at Barrack Lane.
The care navigator is a very important role within the surgery as they are the first person a patient speaks to or sees when they contact the surgery.
General practice is a challenging environment that requires a calm and caring approach with the ability to work under pressure. The ideal candidate will be a highly motivated team player who has customer service/patient experience and offers excellent communication skills.

Main duties of the job
- Sorting all clinical post and prioritising for the GP in terms of actions.
- Extracting all information from clinical letters that needs coding and adding to notes.
- Arranging appointments, referrals, tests and follow up appointments of patients.
- Preparing patients prior to going in to see the GP, taking a brief history and basic readings in readiness for the GP appointment.
- Dipping urine, taking blood pressure, ECGs & phlebotomy.
- Explaining treatment procedures to patients.
- Filing and allocating of test results, with clinical support.
- Working with our care co-ordination team.
- Working to achieve QOF and IIF outcomes.
- Supporting practice team to help patients with their chronic disease management reviews.
About us
Barrack Lane is a busy practice in the town centre of Ipswich, Suffolk. We have a practice population of over 20,000 patients. The practice is based on two sites, the main surgery in the centre of the town, and a branch surgery at Stoke Park.
Barrack Lane is part of a PCN (Primary Care Network) with Ivry Street Medical Centre, in Ipswich, and the two practices cover a population of 32,000 patients. The PCN allows the practices to share resources and offer an enhanced service to our patients. The PCN employs care co-ordinators and social prescribers to work with patients, especially our vulnerable patients, to help them to access the best care. Although you will be employed by the Barrack Lane & Ivry Street PCN you will be based at Barrack Lane.
