Job summary
East Basildon PCN (a collaboration of four GP practices serving approximately 27,000 patients) is seeking to appoint and welcome an enthusiastic and committed ANP to join our primary care network on a full-time basis, for 37.5 hours a week, to deliver a high standard of patient care. The successful candidate will work across the 4 practices within our PCN: Dr Simms & Partners, Matching Green Surgery, Aryan Medical Centre and Felmores Medical Centre.
We envisage that the successful candidate will adopt a highly proactive and collaborative approach in helping to develop this role further and in exploring their own development needs.
Main duties of the job
- Demonstrate courageous and critical thinking and should be a caring, compassionate, committed and experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care. This care may be in several settings: surgery sites, nursing homes, patients homes or by telephone consultation.
- Act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate.
- Provide expert professional advice to patients, carers, and colleagues to ensure the maintenance of clinical excellence.
- Develop new and innovative concepts, models, methods, and practices to deliver new and improved primary care services to meet the needs of the PCN population.

East Basildon PCN is a network made up of 4 surgeries who have 27,000 registered patients. Our vision is to work in partnership with our colleagues and patients to provide the best primary care services in the community. We use SystmOne clinical system across our PCN practices. Our team includes a variety of different professionals who can offer peer support and shared learning. This role will work as part of this large team that includes:

- Nurse practitioners and healthcare assistants

- Be a registered nurse, holding a current registration with the NMC.
- Hold a recognised ANP qualification at masters level.
- Hold an independent prescribing qualification and prescribe safe, effective, and appropriate medication.
Experience

- Be forward-thinking and will strive to develop innovative ideas and new ways of working that address the health needs of the population, while motivating and encouraging others to do the same.
