Job summary

The Beacon Health Group Partnership operates over three sites. The main site, Danbury Medical Centre is based in the village of Danbury. We have two smaller branch sites which are Mountbatten House Surgery (based in North Springfield) and Moulsham Lodge Surgery (based in central Chelmsford).

Due to the size of our Group, the Beacon Health Group is an exceptionally diverse and forward thinking practice. Currently we are recruiting for the role of clinical pharmacist to join our team based across all 3 of our sites. The position will be for up to 37.5 hours per week. Part time and full time options will be considered.

Main duties of the job

  • Working as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas;
  • Being a prescriber, or completing training to become prescribers.
  • Being responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy.
  • Providing specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practice(s) and to help in tackling inequalities;
  • Providing leadership on person-centred medicines optimisation and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services;
  • Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote selfcare;
  • Having a leadership role in supporting further integration of general practice with the wider healthcare teams to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload
  • Developing relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system;
  • Taking a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists, liaison with community pharmacists and anticoagulation.

We also provide Friday extended morning appointments between 7.00am-8.00am, Monday and Wednesday evening appointments between 6.30pm-8.00pm provided by GPs and Nurses.   • Completion of a degree in pharmacy and registration with the General Pharmaceutical Council;
  • Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this;
  • Qualified independent prescriber, or working towards this;
  • Member of the Royal Pharmaceutical Society (RPS).
