Job summary
Little Waltham and Great Notley Surgeryhas an opportunity for a practice-based care coordinator to join our team on afull-time basis.
As a patient care coordinator you will be required to deal with patients and their carers, working closely alongside our multidisciplinary team.
Main duties of the job
Being practice-based, the post-holder will be able to identify patients individual needs, ensuring that they are addressed by taking into account local priorities, health inequalities and population health management risk stratification.
The post-holder will have effective communication skills. A compassionate, caring approach is also essential for this post.
About us
We are a 2-site, friendly training practice with over 20,000 patients.
You will be supported by our team of 10 doctors, 3 nurse practitioners, paramedic, nurses, healthcare assistants, receptionist and admin team.
