Job summary

Ashfield Medical Centre has a fantastic opportunity for an experienced diabetic nurse to join our dedicated team, on a part-time basis for 15 - 22.5 hours per week - days can be flexible. You will be passionate about getting results and making a difference to the lives of our patients with diabetes and we are seeking someone who can provide care, education and support to this ever growing group of patients.

Main duties of the job

  • Undertaking annual diabetic follow up appointments
  • Ensure QOF is up to date for all diabetic patients.
  • Create care plans for diabetic patients.
  • Refer patients to Desmond
  • Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments.
  • Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Be up to date with all necessary training in this field.

Ashfield Medical Centre is a very busy, pro-active and friendly practice based in Beanhill, Milton Keynes. Our 4 GP Partners and staff are a dedicated band and have a great work/life balance ethos.

We are a part of a larger PCN made up of 4 practices in the East of Milton Keynes. We have a great working relationship with the other practices in our PCN.

Essential

  • A relevant nursing degree and Warwick or equivalent diabetic trained.
  • Current NMC registration with an active PIN.
  • Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications specialising in Diabetes.
  • Evidence of clinical skills and experience in treating diabetes with necessary certificates.

  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
  • Knowledge, and experience of its application, in supporting long-term conditions.

  • Great Team Worker with sense of humour!
