Job summary

Rayleigh PCN has an exciting opportunity for a hardworking, enthusiastic and innovative pharmacist to join our primary care network on a part-time basis, for 16 hours per week - the salary for this will be £18,600. The purpose of the role is to provide primary support to general practice staff regarding prescription and medication queries.

Main duties of the job

The role:

  • Performing medicine reconciliation on transfer of care and implementing systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care need of patients in the PCN, working as part of a multi-disciplinary team in a patient-facing role.
  • You will take responsibility for areas of medicines optimisation within the PCN and undertake structured clinical medication reviews to proactively manage patients with complex polypharmacy.
  • Working with all sectors of pharmacy across the health system and its member practices to improve population health, demonstrating improvement in patient outcomes and working to reduce health inequalities across the PCN contributing to improving the efficiency of general practice and delivering a best-in-class service.
  • Ensuring integration with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage GP workload.

Rayleigh and District Primary Care Network (PCN) is made up of six GP surgeries covering a population of approximately 56,000 from the Rayleigh, Hullbridge and Hockley areas. Our PCN serves an area with an interesting population with a varied case mix. Essential

  • Completion of a degree in pharmacy and registration with the General Pharmaceutical Council;
  • Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this;
  • Qualified independent prescriber, or working towards this;
  • Member of the Royal Pharmaceutical Society (RPS);

Experience

Essential

  • Awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice;
  • Ability to demonstrate general pharmaceutical skills and knowledge in core areas;
  • Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions;
  • Ability to demonstrate accountability for delivering professional expertise and direct service provision as an individuals;
