Job summary
We have an exciting opportunity to join our primary care network (PCN) team of allied healthcare professionals working within general practice. GP Healthcare Alliance, a federation of GP practices in Castle Point and Rochford, is hosting an innovative service to recruit an enthusiastic and committed, full-time clinical pharmacist to joint Rayleigh Primary Care Network.
Main duties of the job
You will provide primary support to general practice staff with regards to prescription and medication queries and perform medicines reconciliation on transfer of care and implementing systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care need of patients in the PCN.
You will work as part of a multi-disciplinary team in a patient-facing role and take responsibility for areas of medicines optimisation within the PCN undertaking structured clinical medication reviews to proactively manage patients with complex polypharmacy.
You will provide clinical leadership on medicines optimisation and quality improvement (QI) and manage some aspects of the quality and outcomes framework and enhanced services at practice and PCN level.
You will work with all sectors of pharmacy across the health system and its member practices to improve population health and demonstrate improvement in patient outcomes and work to reduce health inequalities across the PCN to contribute to improving the efficiency of general practice and deliver a best-in-class service.
You will provide support to practice staff with regards to prescription and medication queries, ensure integration with the community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage GP workload.

Rayleigh and District Primary Care Network (PCN) is made up of six GP surgeries covering a population of approximately 56,000 from the Rayleigh, Hullbridge and Hockley areas. Our PCN serves an area with an interesting population with a varied case mix. - Completion of a degree in pharmacy and registration with the General Pharmaceutical Council;
- Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this;
- Qualified independent prescriber, or working towards this;
- Member of the Royal Pharmaceutical Society (RPS);

- Strong experience within primary care;
- Awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice;
- Ability to demonstrate general pharmaceutical skills and knowledge in core areas;
