Job summary

The Bridge PCN has a fantastic opportunity for an experienced community staff nurse to work within our Integrated Community Support Team (ICST) practitioner to join our team on a part-time basis for 24.5 hours per week. This role will be for a fixed term of one year.

Main duties of the job

The Bridge PCN supports two GP practices in the town of Newport Pagnell, ie. Newport Pagnell Medical Centre and Kingfisher Surgery.

Job description

Job responsibilities

  • Work as part of ICST to review A&E submissions and 999 calls for The Bridge population and provide at home support to keep them well.
  • Identify those at risk of admission by visiting and assessing patients not seen in surgery for a year.
  • Visit patients in hospital to secure early and safe discharges.
  • Liaise with hospital nursing staff to prevent admission from A&E to wards.
  • Receive referrals from the 2 member practices of The Bridge PCN to review patients keeping them safe and well at home.
  • Work with other members of the ICST and Social Worker to review patients.
  • Work with the PCN Occupational Therapists and Social Prescribers to review patients.
  • Provide integrated personalised care planning and an active case management system for patients of the PCN over the age of 18 years

  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Insight and understanding of current issues in nursing and the wider NHS.
  • Understanding of evidence based practice.
  • Knowledge of national standards that inform practice eg. NICE guidelines etc.
  • Understanding of equal opportunity and diversity issues.
  • Excellent interpersonal, verbal and written communication skills.
  • Time management and ability to prioritise workload.
  • Self motivated and enthusiastic.
  • Car driver/access to car.
  • Able to analyse data and information, drawing out implications for the patient.
