PCN Care Co-ordinator

Job summary

In partnership with Rochford District Primary Care Network (PCN), GP Healthcare Alliance, a federation of GP practices in Castle Point and Rochford, is hosting an innovative service to recruit a care coordinator to support the PCN additional roles team, providing a central co-ordination role for patient care planning across member practices.

Main duties of the job

This is a full-time post, working 37.5 hours per week, covering Rochford District PCN which is made up of the following practices:

  • Ashingdon Medical Centre
  • The Practice Leecon Way
  • The Puzey Family Practice (base for this role)
  • Wakering Medical Centre

Job responsibilities

This role will require the post-holder to work autonomously, but they will have the support of a multi-professional primary care team made up of doctors, nurses and clinical pharmacists.The post-holder will work within our multidisciplinary team, providing a central co-ordination role for patient care planning, tosupport delivery of the PCN engagement workstream with the local population by initiating contacts as well as developing and managing relationships across the network.
