Job summary

Benfleet Primary Care Network is looking to recruit an advanced nurse practitioner on a fixed term contract until March 2024, although moving over to a permanent contract once we incorporate as a PCN, which is planned for September/October 2023.

Job responsibilities

Main responsibilities:

  • Assess and triage patients, including same day triage and, as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
  • Manage undifferentiated undiagnosed conditions, identifying red flags and underlying serious pathology, and taking appropriate action.
  • Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
  • Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person.
  • To have completed the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.
  • Work within practices, across the PCN and ICS, multi organisational, cross professional and across care pathways.
  • Flexible skill set to adapt to and meet needs of the PCN population and support public health.
  • High level of complex decision making to inform diagnosis, investigation complete management of episodes of care within a broad scope of practice.
  • Manage medical complexity.
  • Lead audit and research projects.
  • Lead QOF within areas of capability.
  • Multi-professional ACP clinical and CPD supervision across all 4 pillars with relevant training.

  • be able to work with a high degree of autonomy and be confident in leading in a variety of settings.
  • demonstrate the ability to lead others, enabling you to use and further develop your teaching and mentoring experience/qualifications.
  • be forward thinking and strive to develop innovative ideas and new ways of working, that address the health needs of the practice, whilst motivating and encouraging others to do the same.
  • be an excellent communicator and collaborator, who looks to embrace opportunities to work and liaise across multi-professional and agency boundaries in the interests of improving patient outcomes.
  • be consistently compassionate and respectful placing a high value on the dignity of patients, relatives, carers and colleagues.
  • demonstrate highly developed clinical skills.
