Kingswood Medical Centre (KMC) in Basildon has a fantastic opportunity for an ANP to join our team on part-time basis, working 15-20 hours a week.

About us

The KMC is a well-established general practice in Basildon providing high quality, compassionate primary care to the local community. Serving approximately 9,500 patients in the area and part of West Basildon PCN, we are aiming to support and strengthen the delivery of primary care services in line with the NHS Long-term Plan.We aim to achieve high standards of patient-centred care as identified for the specific needs of our population, whilst working to support a resilient and appropriately trained workforce.We operate SystmOne clinical system with a good history of QOF achievement and a CQC rating of Good across all domains.We currently have a multi-disciplinary team consisting of four GPs, an ANP, nurses, HCAs, clinical pharmacists, a social prescriber, a first contact physiotherapists and mental health practitioners. As such, you will be joining an enthusiastic team of clinicians and administrators and will be well supported with on-going professional development. The role will contribute to improving quality of patient care across the practice.

Job description

Job responsibilities

The following are the core responsibilities of the advanced nurse practitioner. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  • Develop, implement and embed health promotion and wellbeing programmes
  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
  • Implement and evaluate individual specialised treatment plans for chronic disease patients
  • Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
  • Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • Undertake the collection of pathological specimens
  • Request pathology services as necessary
  • Process and interpret pathology and other test results as required
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
  • Maintain accurate clinical records in conjunction with extant legislation
  • Ensure read/SNOMED CT codes are used effectively
  • Maintain chronic disease registers
  • Develop, implement and embed well woman clinics, well man clinics and travel clinics
  • Develop, implement and embed an effective call/recall system
  • Be responsible for the implementation of an effective immunisation programme
  • Chaperone patients where necessary
  • Prioritise health issues and intervene appropriately
  • Support the team in dealing with clinical emergencies
  • Recognise, assess and refer patients presenting with mental health needs
  • Be an extended and supplementary prescriber, adhering to extant guidance
  • Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
  • Contribute to practice targets (QOF etc.), complying with local and regional guidance
  • Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual
  • Support the clinical team with all safeguarding matters in accordance with local and national policies
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

About the Candidate

Essential

  • Registered Nurse with Nursing and Midwifery Council
  • Either Masters degree required for qualification post December 2020 or a post graduate diploma or degree for Advanced Practice Qualification up to December 2020
  • Qualified Independent Nurse Prescriber on the NMC register.
  • Meets the NMC revalidation requirements in accordance with the NMC Revalidation booklet
  • Meets the standards for the registered ANP working at advanced level
  • Minor illness qualification
