Job summary

South Rural PCNhas an exciting opportunity for a hardworking, enthusiastic and innovative pharmacy technician to join our primary care network. You will be based atNeedham Market Country Practicebut will be required to travel between all of the GP practices within our PCN.This role is offeredon afull-time basis or part-time basis.

About us

South Rural PCN was formed in June 2019 with five like-minded practices wanting to provide a joined up service for local communities. To achieve this we are looking to reshape our clinical teams by developing new and exciting skill mixes, which not only diversify our workforce, but allows us to provide a better care package for our patients.

The PCN provides services to 56,000 patients across rural Suffolk and is headed by 2 proactive, supportive and committed Clinical Directors. Our PCN includes the following practices: Bildeston Health Centre, Constable Country Medical Practice (East Bergholt and Capel St Mary), Hadleigh Health Centre (Boxford), The Holbrook and Shotley Surgery and Needham Market Country Practice.Our practices offer the full range of primary care services. We achieve consistently high QOF and CQC results. We are active and respected members of Ipswich and East Suffolk Clinical Commissioning Group.We offer extended hours appointments to our patients. We provide an enhanced health in care homes service to the 16 care homes across our area.

Job responsibilities

The main responsibilities of the role include:

  • Undertake patient facing and patient supporting roles to ensure effectivemedicines use, through shared decision-making conversations withpatients
  • Conduct medicines optimisation tasks including effective medicineadministration (e.g., checking inhaler technique), support medicationreviews and medicines reconciliation. Where required, utilise consultationskills to work in partnership with patients to ensure they use theirmedicines effectively
  • Support, as determined by the PCN, medication reviews and medicinesreconciliation for new care home patients and synchronise medicines forpatient transfers between care settings and linking with local communitypharmacists
  • Provide specialist expertise, where competent, to address both the publichealth and social care needs of patients including lifestyle advice, serviceinformation and help in tackling local health inequalities
  • Maintain a central role in the clinical aspects of shared care protocols and liaise with specialist pharmacists for more complex patients
  • Support initiatives for antimicrobial stewardship to reduce inappropriateantibiotic prescribing
  • Assist in the delivery of medicines optimisation and management incentiveschemes and patient safety audits;
  • Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. This will be achieved through undertaking clinical audits (e.g., use of antibiotics), supporting quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services;
  • Attend a formal appraisal with your manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed
  • Contribute to public health campaigns (e.g., COVID-19 or flu clinics) through advice or direct care
