Job summary
Saffron Health Partnershiphas an opportunity for a planned care co-ordinator to join our data team. The role is offered on a part-time basis for 20 hours per week, over 4 days, one of which will be a Tuesday working until 6pm.
The data team is made up of 9 experienced and supportive administrators and led by the data specialist manager.
Main duties of the job
The main duties of the role include:
- To use clinical systems (SystmOne) to identify, prioritise and contact patients requiring health checks or recalls.
- To support the practice in quality improvement programmes with a focus on improving patient care.
- To have responsibility for contacting patients with learning disability or severe mental illness prior to their annual health check and support them to attend.
- To pro-actively contact patients with chronic conditions to book reviews and tests when appropriate in a timely manner.
- To contact patients following a recent change in medical circumstances for the purposes of welfare checks and/or follow up appointments.
- Contribute towards improved performance Quality Outcome Framework and Enhanced Services outcomes for the practice.

We are a long-established practice, with a very friendly team, serving patients from our 3 sites in Biggleswade, Sandy and Langford. This role is based at our Biggleswade site, which is under a 5-minute walk from the train station. We also have a staff car park.
