Job summary

Saffron Health Partnershiphas an opportunity for a planned care co-ordinator to join our data team. The role is offered on a part-time basis for 20 hours per week, over 4 days, one of which will be a Tuesday working until 6pm.

The data team is made up of 9 experienced and supportive administrators and led by the data specialist manager.

Main duties of the job

The main duties of the role include:

  • To use clinical systems (SystmOne) to identify, prioritise and contact patients requiring health checks or recalls.
  • To support the practice in quality improvement programmes with a focus on improving patient care.
  • To have responsibility for contacting patients with learning disability or severe mental illness prior to their annual health check and support them to attend.
  • To pro-actively contact patients with chronic conditions to book reviews and tests when appropriate in a timely manner.
  • To contact patients following a recent change in medical circumstances for the purposes of welfare checks and/or follow up appointments.
  • Contribute towards improved performance Quality Outcome Framework and Enhanced Services outcomes for the practice.

About us

We are a long-established practice, with a very friendly team, serving patients from our 3 sites in Biggleswade, Sandy and Langford. This role is based at our Biggleswade site, which is under a 5-minute walk from the train station. We also have a staff car park.

We offer 28 days' annual leave plus bank holidays.

If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

Essential

  • Committed to achieving excellence in patient care.
  • Compassionate and persuasive interpersonal skills with reference to encouraging potentially vulnerable patients to attend reviews and appointments.
  • Comprehensive working knowledge of Microsoft Office, especially Word and Excel.
  • Excellent planning, organisation and communication skills
  • An ability to work on own initiative and manage own workload.
  • Be able to communicate clearly and appropriately with members of the public, colleagues and staff in other agencies verbally, on the telephone and in writing
  • Flexible, adaptable, and resourceful

  • Previous experience of working in primary care, the NHS or social care
