Priory Gardens Surgery is looking to recruit a minor illness nurse to join their clinical team, to providehigh quality practice nursing skills and services, including minor illness, chronic disease management, and health promotion to the practice population. The post-holder will work as an autonomous practitioner providing mentorship to the practice nurses and health care assistants.

Priory Gardens Surgery is a GP practice responsible for 18,000 patients via a GMS contract held with BLMK ICB. Under this contract we employ over 50 staff who work together to deliver GP primary care services to our patients. Our staff skills range from partners, salaried GPs, advanced clinicians such ANPs, pharmacists, paramedics, practice nurses, healthcare assistants, along with a large admin team who supports both clinicians and patients.

The main responsibilities of the role:
- Identify and undertake specialist nursing tasks, to include minor illness consultations for at least 50 per cent of the working week, chronic disease management, home visits and nurse triage.
- Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC).
- Provide care for patients presenting in the minor illness clinic and those that are housebound with minor illnesses and urgent on the day appointments.
- Provide a first point of contact within the practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan.
- Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
- Make professionally autonomous decisions.
- Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
- Provide safe, evidence-based, cost-effective, individualised patient care.
- Refer to the duty doctor as necessary, take responsibility for the delivery of specialist nursing care to include the management and administration of a chronic disease clinic, for example asthma, COPD, diabetes, hypertension, CHD, anti-coagulation, etc., both in the surgery and to those that are housebound, as appropriate to experience and training.
