Job summary
Rayleigh and District PCN is a forward-thinking and friendly primary care network (PCN) looking for two paramedics to join our home-visiting team.This role will be based within Hockley Clinic, 53 Spa Rd, Hockley SS5 4AR but will be working autonomously in the community covering Rayleigh, Hockley and Hullbridge.We are looking for individuals who can work full time, Monday to Friday, 9am 5pm, however other working hours will be considered.
Main duties of the job
The post-holders will work alongside an experienced emergency care practitioner prescriber, undertaking home visits for acute needs as well as for long-term condition management for housebound patients.

- Responding to referrals, providing assessment, diagnosis and treatment at the first point of contact by attending to patients, according to their needs, in the patients homes.
- Accurately triaging and prioritising patients, including in emergency situations and telephone triage, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.
- Evaluating clinical information from examination and history taking to initiating appropriate treatment and/or referral.
- Reviewing the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes.
- Maintaining accurate clinical records in an efficient manner.

- We are looking for someone who is organised and who thrives in a busy environment where no two days are the same. All applicants should:
- Be a registered paramedic with HCPC, with a minimum of 5 years post registration experience or equivalent, ideally with at least 1 year (or equivalent) being in primary care.
- Have completed ENP/ ECP minor injuries and minor illness course level 6.
- To be able to effectively assess, plan, treat and refer patients appropriately and evaluate patient care confidently.
- Have experience of working to protocols and guidelines for patient groups and patient specific directives.
- Exhibit excellent communication skills when dealing with clients: non-verbal and verbal skills required.
- Be able to make clear decisions, with confidence, and communicate these effectively.
- Demonstrate the ability to work within professional boundaries, according to the HCPC code of conduct.
- Have experience of autonomous practice working and multi-professional team working.
- Demonstrate a broad and sound knowledge of complex and common conditions in primary care.
- Ideally, have experience of working in general practice and who would like to take the next step in their career in primary care.
- Ideally, have experience of working with SystmOne.
