Job summary
Aveley, South Ockenden and Purfleet Primary Care Network (ASOP PCN) are recruiting for an advanced nurse practitioner to join their team on a full-time or part-time basis. In this role, the post-holder will initially be focused on getting to know the staff in each practice, and supporting with minor-illness clinics and winter pressure work, before setting-up and leading the obesity/multi-morbidity clinic.
Main duties of the job
The post-holder will be an experienced nurse who, acting within their professional boundaries, will provide care for thepresenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of care.They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills,for patients within the general practice, the multi-morbidity clinics and other Thurrock-wide clinics.

- Clinically examining and assessing patient needs from a physiological and psychological perspective, and planning clinical care accordingly.

- Proactively identifying, diagnosing and managing treatment plans for patients at risk of developing a long-term condition (as appropriate).
- Diagnosing and managing both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
- Prescribing and reviewing medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
