Job summary
Watton Medical Practice is looking to welcome a qualified and experienced clinical pharmacist into our busy surgery, to play a full and active role within our multi-disciplinary team. This role is offered on a full-time basis.
Main duties of the job
The main responsibilities of the role include:
- Working as part of a multi-disciplinary team in a patient-facing role the post holder will take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex poly-pharmacy, especially for older people, people in residential care homes and those with multiple co-morbidities.
- Providing provide primary support to general practice staff with regards to prescription and medication queries. The post holder will help to support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice.
- Providing clinical leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.
- Ensuring that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.

Watton Medical Practice is a busy practice,within a growing South Norfolk market town, not far from the coast and the city of Norwich, and offering affordable housing, a vibrant community and good amenities. Our building is modern, purpose-built and spacious, which allows us to be able to adapt services for our ever-growing list size.
The practice is part of the Breckland Alliance PCN consisting of three practices Grove Surgery and School Lane Surgery from Thetford and Watton Medical Practice. Our GP partners are based across all three sites.
Our clinical team consists of GPs, paramedic, advanced nurse practitioner, practice nurses and healthcare assistant. We are recruiting to grow this team. We enjoy supporting the learning of others including trainee paramedics, physician associates and nursing associates.
